Who Can Make a Referral?

We accept referrals from multiple sources, recognising that different people may be involved in arranging care. Our flexible approach means that referrals can come from the following sources:

Self-Referrals (Individuals)

If you are an adult who believes you would benefit from home care support, you can refer yourself directly to us. There is no need to wait for a healthcare professional to suggest it; if you recognise that you need help, please contact us.

Family Members and Carers

If you are a family member or friend who is concerned about someone's wellbeing or care needs, you can make a referral on their behalf. We will seek the consent of the person for whom you are referring before proceeding.

Social Services and Local Authorities

Care coordinators and social workers from Swindon Borough Council and other local authorities can refer service users to us. We work closely with local authority commissioning teams and accept referrals from local authority-funded care assessments.

General Practitioners (GPs)

GPs can refer their patients to us when they believe home care support would benefit their health and wellbeing. We liaise directly with GPs about patient care needs and receive regular updates on relevant health information.

Hospital Discharge Teams

Step 1

Making the Referral

In person: Visit our office or arrange a meeting with a member of our team to discuss the referral face-to-face.

Step 2

Initial Contact and Discussion

Following your referral, a member of our team will make contact with you (or the person concerned if a self-referral) to discuss the referral in more detail. This initial conversation is informal and exploratory. We will seek to understand:

The person's current situation and care needs.

What support they already have in place.

What they hope to achieve through receiving care.

Any specific preferences, requirements, or concerns.

Funding arrangements (self-funding, local authority-funded, NHS-funded).

Urgency of the need (is care needed immediately, or is there time for assessment?).

This is also an opportunity for you to ask us questions, find out more about our service, and determine whether we are the right provider for you. We will provide you with information about our qualifications, experience, approach, and costs.

Step 3

Pre-Care Assessment and Formal Evaluation

If both you and we believe that Earnest Support Care Ltd could be the right provider, we will arrange a formal Pre-Care Assessment visit at the person's home. This is a comprehensive assessment carried out by our Registered Manager, Claire Louise Lewis, or a trained member of our assessment team.

During this visit, we will conduct a thorough evaluation of the person's care needs, their home environment, their health and social circumstances, and any risks or barriers to independence. Full details of this assessment are provided in our How We Work page.

Following the Pre-Care Assessment, we will produce a written Pre-Care Assessment Report documenting our findings and our recommendation as to whether we can meet the person's needs. This report will be provided to you and to the person concerned within 5 working days.

Step 4

Care Planning and Arrangement

If we have assessed that we can safely and effectively meet the person's needs, we will proceed to develop a bespoke, person-centred care plan. This is created in close collaboration with the person, their family, and any involved healthcare professionals.

The care plan will outline the care and support to be provided, the frequency and duration of visits, the cost, and other relevant details.

We will match the person with suitable care workers and arrange an Introduction Meeting so they can meet their care worker(s) before care commences.

Step 5

Commencement of Care and Ongoing Support

Once all arrangements are in place, care will commence on the agreed date. Our care workers will provide the agreed care and support, and we will maintain close monitoring and communication to ensure everything is working well and to address any concerns.

We will make contact within 2 working days of receiving your referral.

Pre-Care Assessment:

We will arrange the Pre-Care Assessment visit within 5 working days of your initial contact or sooner if needed urgently.

Assessment report:

We will provide the written Pre-Care Assessment Report within 5 working days of the assessment visit.

Care plan development:

If we proceed to develop a care plan, this will be completed within 7 working days of the assessment.

Care commencement:

Care can typically commence within 2–3 weeks of the assessment, depending on care worker availability and the person's preferences.

Urgent cases:

If care is needed urgently, we will do our best to accommodate this. Please tell us if there is urgency so we can prioritise accordingly.
